PSI and MOCs are integral parts of changing and improving processes. Sometimes the MOC and PSI are reviewed with limited knowledgeable personnel in the room. Do you have the right people to review all aspects of PSI and MOCs? At one site, for example, assessments of procedures using all the skill sets in the plant was done. This required a committee of 17 different skill sets! Limited small teams reviewing PSI or MOCs may be a failure point as sections of knowledge and information are missing from the review. Are you reviewing all changes in information or only parts of it?
