Do you have recommendations that haven’t been closed from your last PHA? A Process Hazard Analysis is required at least every 5 years for each PSM covered process and each iteration creates numerous recommendations to be completed. They are a potential source of audit findings and need to be acted on in a timely manner. Keep PHA recommendations under control with regular reviews of aging activities!
